Aligning Your Work with a National Framework for CYSHCN

The National Center for a Systems of Services for Children and Youth with Special Health Care Needs (CYSHCN) developed a worksheet and an accompanying crosswalk to support Title V programs in aligning their work and priorities with a national framework for CYSHCN and their families. 
 
The worksheet presents a starting point for states to identify their current work that aligns with a national framework and begin to consider how they can build on it to improve the system of services for CYSHCN. This tool may support Title V programs in completing their Title V needs assessment and informing future Block Grant application information related to those activities. When thinking about future activities and priorities, consider how activities relevant to a national framework for CYSHCN also crosswalk with the Six Core Indicators.
