The National Center for a System of Services for Children and Youth with Special Health Care Needs (CYSHCN) developed a resource that explores the alignment between the Title V Block Grant National Outcome Measures (NOM) and National Performance Measures (NPM) with areas from a national framework for CYSHCN. Title V programs can use this tool to help understand how their current efforts align and prioritize future efforts and optimize impact in serving CYSHCN. 
This crosswalk encompasses NOMs and NPMs relevant to all infants, children, and adolescents, reflecting an inclusive approach that prioritizes CYSHCN and their families. The criteria used for identifying intersecting NOMs/NPMs with areas from a national framework for CYSHCN, as well as input from subject matter experts at the National Center for a System of Services for CYSHCN, including Family Voices, state Title V CYSHCN programs, and other national partners. While the relationships outlined in this crosswalk are informed by these factors, we recognize that interpretations may vary among state Title V programs and their partners.
