Strategy Measures for Medical Home NPM

This resource provides a list of Evidence-based or -informed Strategy Measures (ESMs) related to the medical home National Performance Measure (NPM) collected from the Health Resources & Services Administration (HRSA) Maternal and Child Health Bureau (MCHB) Title V Information System (TVIS). ESMs for the overall medical home NPM as well as three subcomponents of the medical home NPM are featured and pulled from the 2025 Applications/2023 Annual Reports. Categorization was completed based on a review of National Survey of Children’s Health (NSCH) questions which aligned with each component of the medical home NPM.
