The National Center for Adolescent and Young Adult Health and Well-Being is here to support adolescents, their families/caregivers, and the professionals who care for them by seeking opportunities to provide comprehensive care and support that meets adolescents’ needs where they are. The National Center for Adolescent and Young Adult Health and Well-Being is funded through a 5-year cooperative agreement with the Health Resources and Services Administration (HRSA).
What We Do
Adolescence and young adulthood is a time of intense developmental change that presents unique health and social needs, while also a time of remarkable resilience, growth and opportunity. The goal of the National Center for Adolescent and Young Adult Health and Well-Being is to increase the capacity of states, territories and tribal organizations to integrate systems of care, including health, school and community systems, to promote adolescent and young adult health and well-being.
To accomplish this goal, the National Center for Adolescent and Young Adult Health and Well-Being will:
- Establish a cross-sector alliance comprised of partners in health, school and community systems that will provide national leadership and identify avenues to promote systems integration.
- Create a national blueprint for comprehensive systems integration for adolescent and young adult health.
- Provide targeted technical assistance (TA) to learning collaboratives for up to 9 state, territory and/or tribal organization teams working to implement cross-sector systems integration models using the national blueprint.
- Provide technical assistance to State Adolescent Health Coordinators and Title V agencies who have selected performance measures related to improving adolescent and young adult health.
- Promote implementation of comprehensive systems integration models.
