Fillable State Data Tables from the National Survey of Children’s Health
The purpose of the table templates is to guide Title V programs in thinking about their own state’s performance on different adolescent health and wellbeing measures. The tables link to state data in the National Survey of Children’s Health (NSCH) and are fillable by the user. Findings from the table can be used as a starting point to help identify areas for improvement and where to align current work to advance improvements in adolescent health and wellbeing.
Before completing the tables, please check out the corresponding report, with the key national findings and measures. Please note that the report uses 2021/2022 data while the tables in the report use 2022/2023 data, which is the most current data at the time of publication of this resource. Any year of survey data can be used in this resource, simply by modifying the years found here.

Data Brief: Adolescent Health and Wellbeing Measures and Estimates from the National Survey of Children’s Health
This adolescent data brief provides an overview of national level data collected from the National Survey of Children’s Health (NSCH). It is intended for use by state Maternal and Child Health (MCH) and Children and Youth with Special Health Care Needs (CYSHCN) programs as they conduct their Title V Maternal and Child Health Block Grant needs assessments, but can be used at any time to inform states' overall approach to monitoring adolescent and young adult (AYA) health and wellbeing. The current data brief published in 2024 uses the 2021/2022 NSCH data.
Report: Title V Block Grant Review of Adolescent Health Performance Measures Strategies
This report presents findings from 2024 Title V Block Grant Applications. It examines adolescent-related national performance measures (NPMs) and state performance measures (SPMs) across states, summarizes distinctive NPM and SPM cross-sector partnership strategies, and provides state examples of standout strategies involving cross-sector partnerships. These Title V activities findings reveal important ideas for cross-sector partnerships with sister state agencies, schools, community organizations, and more. This report is intended for use by state Maternal and Child Health (MCH) and Children and Youth with Special Health Care Needs (CYSHCN) programs as they conduct their Title V MCH Block Grant needs assessments.
