Strength-based Screening and Discussion

Before you screen, it is important to have identified quality referral resources to share with patients and families to meet needs identified in their assessment. Get to know your local community resources by creating relationships with partners. Building relationships with local community support resources takes time.

1. These are a few national organizations that have local supports, FindHelp, Circles of Security, Help Me Grow, Postpartum Support International, and United Way 2-1-1.
2. Engage families/family advisors for feedback to ensure quality referral resources that meet families' needs.
3. Connect with your local AAP Chapter to get strengthen relationships with community support resource networks.
4. Get to know the community you serve by finding out more about your patient demographics. Here are a few resources can help you understand your community Annie E. Casey Foundation KidsCount Data Center Opportunity Atlas, and Robert Wood Johnson Foundation County Health Rankings.
