Before you screen, it is important to have identified quality referral resources to share with patients and families to meet needs identified in their assessment. Get to know your local community resources by creating relationships with partners. Building relationships with local community support resources takes time.

- Engage families/family advisors for feedback to ensure quality referral resources that meet families' needs.
- Connect with your local AAP Chapter to get strengthen relationships with community support resource networks.
