A medical home is not a building or a place—it is an approach to providing comprehensive and high-quality primary care with a focus on building and developing partnerships with families, pediatric clinicians, early childhood professionals, community organizations, educational systems and other key agencies within the system of care. A patient/family-centered medical home should be:
- Accessible
- Family-centered
- Continuous
- Comprehensive
- Coordinated
- Compassionate
- Culturally effective
Why is it Important?
According to the 2018-2019 National Survey of Children’s Health, less than 48% of families indicated that their child received coordinated, ongoing, comprehensive care within a medical home and less than 18% received care in a well-functioning system. The need for a patient/family-centered medical home and improvements to the system of care is especially important for children and youth with special health care needs and their families who oftentimes require significant care coordination and care integration.
Studies have shown that the medical home model of care:
- Reduces the impact of healthcare costs
- Improves healthcare quality
- Increases family experience and satisfaction
- Increases clinician experience and satisfaction
