A quality improvement (QI) approach supports the testing and implementation of new processes or practice changes. In this project, QI measures were identified and used to assess the consistency and reliability of screening, assessment, and follow-up for anxiety and depression. The AAP ECHO team developed a data collection tool to capture information on these measures.
Measures
This QI measures grid, produced by the AAP ECHO team, along with the data collection tool, can be adapted to your practice and setting.

Measure 1: Administer behavioral / social / emotional screening |
% of patients 8-21 years old who have had an age- appropriate practice-standardized, behavioral/social/emotional screening administered at the most recent health supervision visit or at another health visit within the last year consistent with AAP recommendations. |

Denominator: All patients ages 8-21 seen in practice for health supervision or other health visit whose records are viewed. |
Patient record/record review tool |
10% improvement over baseline or 95% |
2. Is there documentation in the patient’s medical record indicating a behavioral / social / emotional health screening tool was administered at the most recent health supervision or other appropriate health visit? |

Measure 3: Provide a brief intervention at a recent health supervision or other health visit |
% of patients / families ages 8-21 with a positive screen that receieved a brief intervention for anxiety or depression during the most recent health supervision visit or at another health visit in the last year. |

Patient record / Record review tool |
10% improvement over baseline or 95% |
5. Is there documentation in the medical record indicating that a brief intervention was provided following a positive screen during the visit? |
